COVID-19 Questionnaire Screening

If you or the minor you are signing for is unable to affirm the following questions, we will not be able to accommodate you at Terrapin Adventures.

If any answers change between when you fill this document out and when you arrive at Terrapin Adventures you must inform a staff member immediately and we will not be able to accommodate you. We will issue you a FULL REFUND.

We take the health and safety of everyone who comes out to Terrapin Adventures very seriously, thank you for your cooperation.

Please read each question carefully.

Have you (or any of the minor(s) you are signing for) experienced any of the following symptoms in the last 48 hours: • fever or chills • cough • shortness of breath or difficulty breathing • fatigue • muscle or body aches • headache • new loss of taste or smell • sore throat • congestion or runny nose • nausea or vomiting • diarrhea

Within the past 14 days, have you (or any of the minor(s) you are signing for) been in close physical contact (6 feet or closer for at least 15 minutes) with a person who is known to have laboratory-confirmed COVID-19 or with anyone who has any symptoms consistent with COVID-19?

Are you (or any of the minor(s) you are signing for) isolating or quarantining because you may have been exposed to a person with COVID-19 or are worried that you may be sick with COVID-19?

Are you (or any of the minor(s) you are signing for) currently waiting on the results of a COVID-19 test?
