Camper Information Form
The purpose of this form is to collect important personal information about the registered child and their parents/guardians/emergency contacts. Each child registered must have their own information form.
Please provide the child's information in this section.
Please provide the address that the child will be residing at during camp. This may be different than their normal home address.
Please provide parent/guardian information in this section.
Please provide two non-parent emergency contacts in this section. In the event of an emergency, the camp will first attempt to contact the child's parents/guardians.
I attest that the information provided in this document is true and correct. I acknowledge that if any of the above information changes before camp that I must submit a new form.
